- Complete Fellow Interest Form
If you have been accepted into an SMSNA Fellowship Program:
- Complete a Fellow Information Form
- Complete an initial self-assessment which will be completed again at the mid-term and end of the program. This will guide learning objectives throughout the year and measure skill and knowledge acquisition.
